We are looking for a Senior Social Support Worker to join our Social Support team and help shape high-quality, person-centred programs that make a real difference every day.  This is a dynamic blended role where you will combine hands-on client support with leadership, coordination and service oversight responsibilities across multiple sites.

Working closely with the Social Support Team Leader, you will play a key role in supporting client wellbeing, strengthening engagement, contributing to care planning and driving continuous improvement across programs.

Your week will include both direct client-facing work and dedicated time for program coordination, staff support and service quality activities, ensuring consistency and excellence across all Social Support services.

Key Responsibilities

  • Deliver person-centred Social Support programs (centre-based and community)
  • Contribute to intake, assessments and care planning
  • Monitor client wellbeing and escalate concerns as required
  • Support scheduling and continuity of services across sites
  • Build relationships with clients, families and carers
  • Provide guidance and support to Social Support Workers
  • Support oversight of program delivery and service quality across sites
  • Escalate risks, incidents and service issues to the Team Leader
  • Contribute to documentation, care plan reviews and continuous improvement
  • Providing on-call phone support fortnightly on a roster basis.

To be considered for this rewarding role you will have:

  • Relevant tertiary qualification relevant to Community Services/Aged Care or Disability
  • Understanding of what is involved in working with aged care
  • Strong communication and relationship-building skills
  • Experience providing informal leadership or team support
  • Strong organisation and time management skills
  • Current driver’s licence and willingness to travel between sites

The fine print

  • Successful candidates will be required to undergo a National Police Check, International Police Check (if applicable), hold a valid WWCC.
  • Full driver license and travel between sites required.
  • Experience and ability to drive DPV Health Buses (11-seater)
  • Level 2 First Aid Certificate
