Join Avivo’s small, supportive nurse team and make a genuine difference in your community. This is a 12-month fixed term contract, working 3 days per week (45 hours per fortnight) — giving you consistency, with space to balance life outside of work.

You’ll have the autonomy to plan your days, build meaningful relationships with customers, and spend your time where it matters.

In this role, you’ll support customers across Geraldton and within the regional area through home visits, health care planning, and clinical training for support workers — with variety in your day and the backing of a highly experienced team.

What you'll do:

  • Deliver hands-on clinical care including wound, tracheostomy, PEG, catheter (suprapubic & indwelling) and diabetes management
  • Review and update personalized health care plans
  • Mentor and guide support workers and external agencies in clinical skills
  • Work closely with families, coordinators and other health professionals
  • Travel across Geraldton and within the regional area

What you'll bring:

  • Current AHPRA RN registration
  • 3+ years post-graduate clinical experience
  • 2+ years in Community Care Services
  • Experience in a Senior Clinician or Team Leader role
  • Strong clinical judgement and documentation skills
  • Confidence supporting and training others
  • First Aid/CPR & NDIS Worker Screening (or willingness to obtain)
  • Valid WA Driver’s Licence
