Job description

Healthii People is pleased to be supporting Advance Diversity Services in support of recruitment for an experienced Care Partner (Support at Home Packages) to join them on casual basis. The Bilingual Care Partner will support and work with individuals with their specific needs and goals to maximize their choices and independence. This role is responsible for aligning quality care plans with client-specific needs and effective service delivery through the Consumer Directed Care Model (CDC), within a person-centered, wellness & enablement approach.

Your day-to-day will include:

  • Conduct client assessments to understand individual goals, needs, and preferences
  • Develop tailored care plans that support clients to live independently and confidently at home
  • Possess cultural awareness and sensitivity
  • Collaborate closely with families, healthcare professionals, and service providers to achieve positive outcomes
  • Support clients in making informed choices that promote dignity, independence, and empowerment
  • Develop and implement appropriate goal-directed and flexible care plans in conjunction with consumers/carers in response to needs, preferences and budget, Workforce Experience under the Consumer Directed Care principles.

What You'll Need:

  • Tertiary qualifications in social work, aged care, allied health or demonstrated competence/ experience.
  • Demonstrated experience and have worked in assessment, case management and reporting roles within the community aged care sector.
  • Proven knowledge of consumer-directed care and understanding of community aged care services and their requirements.
  • Strong communicator, both written and verbal. Be able to speak fluently in English. Bilingual candidates strongly considered (Arabic, Greek or Italian preferred).
  • Current National Criminal History Record Check 
  • Current NSW driver’s license and have access to a car for work with the ability to travel regularly.
